ABSTRACT

Objective:Based on the framework of 5W1H analysis, to establish the continuous nursing project by delphi expert consultation method for children with congenital imperforate anus after discharge.Methods:This study is observational. Through the literature review, qualitative interview and group discussion to establish the expert correspondence questionnaire from April 2021 to March 2022. Delphi consultation was applied to experts for two rounds of expert consultation, to evaluate the initiative, authority and coordination of expert opinions.Results:Two rounds of expert consultation were carried out with 20 experts.The response rates of two rounds of expert questionnaires were 19/20 and 20/20 respectively, the expert authority coefficients were 0.873 and 0.893 respectively, and the Kendall′s coefficients were 0.311 and 0.405 respectively. The coordination of expert opinions was statistically significant ( P<0.05); After two rounds of expert consultation, it was finally determined that the continuing care plan for children with anal atresia included three first-level indicators (composition and responsibilities of continuing care personnel, implementation methods of continuing care, and implementation content of continuing care), 23 second-level indicators, and 46 third-level indicators. Conclusions:This study have high enthusiasm and authority of experts, and concentrated expert opinions on various indicators. The results are reliable, scientific and comprehensive, which will provide a basis for clinical medical staff to carry out the continuous nursing of children with congenital imperforate anus.

ABSTRACT

Objective:To investigate the effects of PDCA method on improving the accuracy of the Autar Deep Venous Thrombosis (DVT) Scale used by nurses to assess the risk of deep venous thrombosis in surgical patients.Methods:Applied the way of PDCA, namely, raising questions, analyzing reasons, implementing measures, feeding back effects to professional explanate the difficulty in using the scale, moreover, training the nurses used level education method between August 2017 and December2018.The accuracy and consistency of the scale were compared before and after interventions.Results:A total 396 patients were evaluated in the study. There were significant differences in the consistency of evaluation among hospitalized, post-operative and discharged patients after intervention compared with before intervention ( P< 0.0167).The consistency of assessment of team members and responsible nurses increased from 68 to 120 cases, and the consistency of assessment reached 90.91%. The Kappa consistency of assessment of responsible nurses and team members increased from 0.354 before intervention to 0.879 after intervention. The effect was remarkable. Conclusions:PDCA cycle method is based on the problems found in clinical practice. It clarifies the professional terms and rules used in the scale, improves the accuracy and consistency of the Autar DVT scale used by nurses, correctly reflects the risk of thrombosis, ensures the safety of patients, and is worthy of promotion and reference.

ABSTRACT

Dysfunction of the Hippo pathway enables cells to evade contact inhibition and provides advantages for cancerous overgrowth. However, for a significant portion of human cancer, how Hippo signaling is perturbed remains unknown. To answer this question, we performed a genome-wide screening for genes that affect the Hippo pathway in Drosophila and cross-referenced the hit genes with human cancer genome. In our screen, Prosap was identified as a novel regulator of the Hippo pathway that potently affects tissue growth. Interestingly, a mammalian homolog of Prosap, SHANK2, is the most frequently amplified gene on 11q13, a major tumor amplicon in human cancer. Gene amplification profile in this 11q13 amplicon clearly indicates selective pressure for SHANK2 amplification. More importantly, across the human cancer genome, SHANK2 is the most frequently amplified gene that is not located within the Myc amplicon. Further studies in multiple human cell lines confirmed that SHANK2 overexpression causes deregulation of Hippo signaling through competitive binding for a LATS1 activator, and as a potential oncogene, SHANK2 promotes cellular transformation and tumor formation in vivo. In cancer cell lines with deregulated Hippo pathway, depletion of SHANK2 restores Hippo signaling and ceases cellular proliferation. Taken together, these results suggest that SHANK2 is an evolutionarily conserved Hippo pathway regulator, commonly amplified in human cancer and potently promotes cancer. Our study for the first time illustrated oncogenic function of SHANK2, one of the most frequently amplified gene in human cancer. Furthermore, given that in normal adult tissues, SHANK2's expression is largely restricted to the nervous system, SHANK2 may represent an interesting target for anticancer therapy.

ABSTRACT

Objective To investigate the effects of the multidisciplinary cooperative management intervention model on the self‐management efficacy of patients with ileostomy irritant dermatitis. Methods Among the patients with irritative dermatitis in the ileostomy section of the Anorectal Surgery and Ostomy Clinic of Hwamei Hospital, the Chinese Academy of Sciences, 72 patients, who visited the clinic from August 2016 to August 2018, met the inclusion criteria and were finally included in the study. According to the random number table method, 36 patients each were assigned to the control and observation groups respectively. There were no significant differences in age, sex and education between the two cohorts. The control groups were compared. The control group was treated with routine nursing methods. The observation group underwent multidisciplinary nursing intervention that was based on the routine measures of the control group. The Chinese version of the cancer self‐management efficacy was used as the questionnaire survey that was administered before and after treatment. The results were compared and analyzed between the patient groups. The independent sample t‐test was used to compare the results between the groups, and the paired sample t‐test was used for intra‐group comparisons. Results After intervention, the total scores of self‐management efficacy and positive attitude, self‐decompression and self‐decision scores were higher in the observation group than in the control group (t=4.192, 3.095, 4.123, 2.267, P<0.05). The scores of self‐administration efficacy and each dimension of the two groups were higher than those before intervention, and the difference was statistically significant (P<0.01). After the intervention, 3 patients (10.0%) in the observation group with recurrent dermatitis and 13 patients (43.3%) in the control group. The recurrence rate of the observation group was significantly lower than that of the control group (χ2= 8.357, P<0.05). Conclusion Multidisciplinary cooperative management can improve the self‐management efficacy of patients with irritating dermatitis and reduce the incidence of this condition.

ABSTRACT

Objective To study the protective effect of Panax notoginseng saponins(PNS)and its components Rg1 and Rb1 on oxygen-glucose deprivation/reoxygenation(OGD/Reox)-induced tight junction damage. Methods Anaerobic box were used to induce OGD in HUVEC cells for 6 h followed by reoxygenation for 24 h. Transepithelial/endothelial electrical resistance(TEER)and cell permeability were detected,immunefluorescence was used to observe the ZO-1 and claudin-5 protein expression. Results PNS 20,40 mg/L and ginsenoside Rb1 significantly inhibited the OGD/Reox-induced decreased tight junction resistance,and the increased cell permeability(P< 0.05). PNS 20,40 mg/L and ginsenoside Rb1 partly restored the inter-cellular tight junctions which were regularly arranged on the cell membrane, and the cells displayed cobble stone-like arrangement. Conclusions PNS ameliorates ischemia-induced vascular endothelial cell tight junction damage via MMP-9 and VEGF/VEGFR2 signaling pathway. Rb1 is one of the effective monomer components.

ABSTRACT

Objective: To explore the lipid-lowering effect and potential mechanism of the couplet medicines of Flastem Milkvetch Seed and Tribulus terrestris on hyperlipidemia rats. Methods: Forty-eight Sprague-Dawley rats were randomly divided into 6 groups, normal control group, high-fat model group, simvastatin group, Tribulus terrestris group, Flastem Milkvetch Seed group, and couplet medicines of Flastem Milkvetch Seed and Tribulus terrestris group, with 8 rats in each group. The normal control group was fed with basal diet, and the other groups were fed with high-fat diet to establish a hyperlipidemia rat model. At the same time, different group rats were treated with different drug respectively. 6 weeks later, the body weight and visceral index of the rats were measured. Levels of serum cholesterol (TC), triglyceride (TG), high density lipoprotein cholesterol (HDL-C), and low density lipoprotein cholesterol (LDL-C) were detected by automatic biochemical analyzer. The mRNA and protein expression of HMG coenzyme A reductase (HMGCR), cholesterol 7-alpha hydroxy-lase (CYP7A1) and low density lipoprotein receptor (LDL-R) were detected by RT-PCR and western blot. Results: Compared with model group, the serum levels of TC, TG, and LDL-C were significantly decreased in all Chinese medicine groups, and the serum level of HDL-C was significantly increased in Flastem Milkvetch Seed group and couplet medicines of Flastem Milkvetch Seed and Tribulus terrestris group. Moreover, the mRNA and protein expression of HMGCR, CYP7A1, and LDL-R in rat liver also significantly increased in all Chinese medicine treatment groups. And couplet medicines of Flastem Milkvetch Seed and Tribulus terrestris group shows more significantly effect. Conclusion: Couplet medicines of Flastem Milkvetch Seed and Tribulus terrestris significantly regulate the serum lipid levels on hyperlipidemia rat model, and its mechanism may be related to the regulating expressions of HMGCR, CYP7A1, LDL-R gene.

ABSTRACT

Objective To investigate the effects of early sucking a lollipop and pacifier to improve the recovery of gastrointestinal function in infant after gastrointestinal operation. Methods The infants after gastrointestinal operation were divided into four groups according to the random number table method, namely group Ⅰ, Ⅱ, Ⅲ and Ⅳ. Four groups were given different interventions in addition to routine nursing care after anesthesia awake, group Ⅰsucked a lollipop,while groupⅡsucking pacifier. Intervention time of 2 groups were 20-30 minites. GroupⅢsucked the pacifier 20-30 minites first, then sucked a lollipop 20-30 minites. Each group was given one intervention every 4 hours until infant eated. Group Ⅳ was given no interventions. Bowel sounds recovery time, first bowel movement time for the first time were compared. SPSS 17.0 software was used for statistical analysis. Results A total of 82 infants completed the clinical observation. Bowel sounds recovery time of group Ⅰ,Ⅱ,Ⅲ were (28.54±9.93), (34.10±9.43), (28.50±9.52) h, while first bowel movement time were (50.02±11.60), (57.65±15.28), (49.97±15.46) h, which were earlier than group Ⅳ whose bowel sounds recovery time and first bowel movement time were (39.55 ±11.22) h, (65.43 ±14.78) h. The bowel movement time and first bowel movement time of group Ⅲ were earlier than that of groupⅡ (P0.05). Conclusions Infants early sucking a lollipop and pacifier after gastrointestinal operation help gastrointestinal function recovery, but the lollipop is superior to the pacifier, lollipops and no interaction between pacifier.

ABSTRACT

Objective To explore the optimum flow shear stress and mass transport for the construction of tissue-engineered bone.Methods The β-tricalcium phosphate (β-TCP) scaffolds seeded with human bone marrow-derived mesenchymal stem cells (HBMMSCs) were cultured in perfusion bioreactor.When the same flow rate was applied,the flow shear stress was separately 1×,2× and 3×.When the same flow shear stress was applied,the flow rates were separately 3 ml/min,6 ml/min and 9 ml/min.Cell proliferation was measured by MTT method.The construction of tissue-engineered bone was evaluated by measuring alkaline phosphatase (AKP) activity,secretion of osteopontin (OP) and osteocalcin (OC),and the mineralization of extracellular matrix (ECM).The flow shear stress and the mass transport were obtained using computational fluid dynamics.Results When the flow rate was same,the most cell proliferation was found in 2× group.The AKP activity and secretion of OC was higher in 2× and 3× groups than in those in 1× group.After 28days,the highest amount of mineralization of ECM was found in 3× group.When the flow shear stress was same,the AKP activity was highest in 6 ml/min group.After 28 days,secretion of OC and formation of mineralized ECM was highest in 3 ml/min group.When the flow rate was same,the flow shear stress was separately 0.004-0.007 Pa,0.009-0.013 Pa and 0.013-0.018 Pa.When the flow shear stress was same,the flow rate was separately 0.267-0.384 mm/s,0.521-0.765 mm/s and 0.765-1.177 mm/s.Conclusion When the tissue-engineered bone was constructed,0.013-0.018 Pa flow shear stress and 0.267-0.384 mm/s mass transport velocity could improve the construction of the tissue-engineered bone in vitro.

ABSTRACT

<p><b>OBJECTIVE</b>To prepare the polyclonal antibody to amelogenin.</p><p><b>METHODS</b>The fetal porcine dental enamel was collected. Enamel matrix protein was extracted in 4M guanidine HCl (pH 7.4) with protease inhibitors present. Polyacrylamide gel filtration was included to isolate amelogenin from the initial dissociated extraction. The purified amelogenin conjugated with or without complete/incomplete Freund's adjuvant was then used to immunize the rabbits subcutaneously or intravenously. The specific IgG antibody was further purified by DE-52 cellulose. The working concentration of IgG antibody was determined through ELISA test.</p><p><b>RESULTS</b>The Gel filtration showed that amelogenin components is at molecular weights of 15 kD and 13 kD apparently, which was consistent with those described before. The ELISA results showed that the working concentration for IgG was 1:1000.</p><p><b>CONCLUSION</b>The antibody prepared in this study can be used for the detection of amelogenin.</p>

ABSTRACT

<p><b>OBJECTIVE</b>The purpose of this study was to construct a eukaryotic expression vector for human amelogenin (AMG).</p><p><b>METHODS</b>PCR was performed to amplify the AMG encoding region. Amplified fragments for human AMG were recovered and inserted into eukaryotic expression vectors PsecTaq2A. The recombinant plasmid PsecTaq2A-AMG was constructed and their positive clones were identified.</p><p><b>RESULTS</b>1. Amplified products were checked by electrophoresis and the results were satisfactory. 2. The recombinant plasmid PsecTaq2A-AMG was analyzed by restriction endonuclease mapping and DNA sequencing. The results of sequencing were consistent with those from GenBank.</p><p><b>CONCLUSION</b>The recombinant plasmid PsecTaq2A-AMG was successfully constructed with properly inserted DNA sequence encoding mature amelogenin.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the relationship between interleukin-1 (IL-1) gene polymorphisms and the susceptibility of chronic periodontitis in Uighur minority in Xingjiang province of China.</p><p><b>METHODS</b>The buccal swabs were collected from 41 severe chronic periodontitis (CP) patients, 43 moderate CP patients, 49 mild CP patients and 92 healthy controls. DNA was extracted from these buccal swabs. Genotypes of the IL-1A-889/NcoI and IL-1B+3954/TaqI were determined by sequence specific primers-polymerase chain reaction(SSP-PCR) and PCR-restriction fragment length polymorphism(PCR-RFLP). Then distribution of genotypes for IL-1A-889 and IL-1B+3954 were compared among the different groups.</p><p><b>RESULTS</b>(1) There were no significant differences in the distribution of IL-1A-889 among severe CP patients, moderate CP patients, mild CP patients and healthy controls. (2) Frequencies of allele 2 for IL-1B+3954 were higher in severe CP patients than in healthy controls, and the difference was statistically significant. But there were no such significant differences either between moderate CP patients and healthy controls or between mild CP patients and healthy controls.</p><p><b>CONCLUSION</b>These results suggest that IL-1B+3953 allele 2 may be a risk indicator for the susceptibility to severe chronic periodontitis in Uighur minority in Xingjiang of China.</p>

ABSTRACT

Objective To investigate the qualitative diagnostic value of MRCP in patients with benign obstructive jaundice.Methods MRCP and conventional MR images were performed in 128 patients with benign obstructive jaundice on a superconductive MR scanner (Plilips medical systems Inc. Eclipse. 1.5T) . Final diagnosis was confirmed by surgical and histopathological findings in 123 patients and other 5 patients by follow-up.Results The accuracy of location diagnosis of MRCP was 100%; the qualitative diagnosis was 95.3%. The specificity, accuracy, sensitivity of the deadwood-sign for diagnosis of benign obstructive jaundice were 100%, 95.3%, 85.7% respectively. Conclusion The deadwood-sign was a specific indicator in MRCP of benign obstructive jaundice.

ABSTRACT

Objective To evaluate the security and biocompatibility of a new kind of absorbable high-strength PDLLA-CPP composite, which is used for internal fracture fixation. Methods According to the national standard of the procedure of appraise in food security and toxicology, micronucleus test of bone marrow cell, spermatic aberration test, chromosomal aberration test, and teratogenic test were measured with IP maximal tolerance dose of maceration extract from PDLLA-CPP composites. Meanwhile, chondrocyte culture with PDLLA-CPP composite was used to evaluate the effect of PDLLA-CPP composite on cartilage cell proliferation. Results Micronucleus test of bone marrow cell, spermatic aberration test, chromosomal aberration test, and teratogenic test were not affected by IP maximal tolerance dose of PDLLA-CPP maceration extract. PDLLA-CPP composite also had no cytotoxicity on cartilage cell proliferation. Conclusion PDLLA-CPP is a new and safe medical material with good biocompatibility and absorbability.
